When you join your new Medicaid Reform health plan, you will select a doctor to be your Primary Care Provider (PCP). Your PCP will provide your health care and refer you to other doctors if you need them. If you like the doctor you have now, and want to stay with that doctor, you must pick a Reform health plan that includes that doctor. You can call a Choice Counselor or ask your doctor to see what Medicaid Reform health plans your doctor is in. If your doctor is not in a Medicaid Reform health plan, call a Choice Counselor at the Helpline: 1-866-454-3959; TDD 1-866-467-4970
Think About the Services You and Your Family Need
Not all health plans are the same. Look for the plan that best serves you or your eligible family member's medical needs. Remember - Each family member eligible for Medicaid may be in a different health plan and have a different PCP. Regardless of the health plan you pick, all plans must provide certain services.
- All medically necessary services for children and pregnant women
- Physician and physician extender services
- Hospital inpatient care
- Emergency care
- Child Health Check-Up (CHCUP) and other services to children
- Maternity care and other services to pregnant women
- Transplant services
- Medical/drug therapies (chemo, dialysis)
- Family planning
- Outpatient surgery
- Laboratory and radiology
- Transportation (emergency and non-emergency)
- Outpatient mental health services
- Hospital outpatient services
- Durable medical equipment
- Home health care
- Prescription drugs
- And more
Things to Consider Before You Pick Your Health Plan or Contact a Choice Counselor:
You should answer these questions for each Medicaid eligible person in your household prior to calling or visiting a Choice Counselor. This will allow the Choice Counselor to better help you pick a plan.
- Where do you go for medical care most of time?
- Doctor's Office
- Clinic
- Emergency Room
- Community Health Clinic
- Mental Health Clinic
- Other
- What are the names of your doctor and clinic/hospital where you receive your medical services?
- If possible would you like to continue to see your current doctor?
- Are you currently with a managed care organization?
- When you need medical care, how do you travel to see a doctor?
- Are you seeing a specialist?
- What is your most important health care need?
- What prescribed medicines are you taking?
- Is there a language other than English you would like your doctor to speak or do you have special communications needs (i.e. sign language)
- If you are employed, the State may help you pay the employer insurance. Are you interested?
- If you have a chronic medical condition, would you like a health plan that serves these medical needs?
