Enroll in Your Health Plan

It's easy. With Reform you have three ways to enroll:

Call-In

Please have your Medicaid ID or Gold Card number, Social Security number, and birth dates for each person you are enrolling before calling.

In-Person

Group counseling meetings or one-on-one counseling in your area - Call the Helpline to find out about scheduled group meetings or to schedule a one-on-one meeting.

Anyone needing special accommodations under the Americans with Disabilities Act of 1990 needs to call the Helpline at least seven days prior to the meeting.

If you are disabled or are not physically able to travel, a Choice Counselor will come to you. Call the Helpline to schedule a meeting.

Mail-In

If you know which Reform health plan you want, fill out an enrollment form and mail it to a Choice Counselor. All Mandatory Group beneficiaries will receive a Reform Information packet with an enrollment form in the mail. If you did not receive one, and want to enroll, call the Helpline.

After you enroll, you will receive a letter that will confirm the health plan you picked.
